The agreement that every doctor will have with you is that it is your choice. You need to feel comfortable with the surgeon and the decision to go ahead with the surgery. If you meet a surgeon and are not satisfied, you can be referred to another surgeon of your choice or you will be refunded the total amount, minus the cost of the consultation (in the case that you had one).

No, you do not have to learn Portuguese. Most doctors are fluent in English and other languages. In case they do not speak your language they will have staff who will translate for you free of charge.
